Zadanie skierowane by\u0142o do 36 os\u00f3b z niepe\u0142nosprawno\u015bciami, zamieszkuj\u0105cych na terenie miasta Pozna\u0144 oraz powiat\u00f3w pozna\u0144skiego, krotoszy\u0144skiego, chodzieskiego i k\u0119pi\u0144skiego. \u00a0Wsparcie by\u0142o co do zasady skierowane do os\u00f3b w kryzysach psychicznych, obj\u0119tych pomoc\u0105 w ramach \u015arodowiskowych Centr\u00f3w Zdrowia Psychicznego i by\u0142o elementem kompleksowego wsparcia \u015brodowiskowego \u015bwiadczonego w Centrach. Wsparciem obj\u0119to grup\u0119 os\u00f3b trafiaj\u0105cych do Centr\u00f3w jak\u0105 by\u0142y osoby cz\u0119\u015bciowo niesamodzielne, z ograniczeniami w poruszaniu si\u0119 b\u0119d\u0105cymi skutkiem niepe\u0142nosprawno\u015bci ruchowej, nabytych w wyniku zaburzenia psychicznego nadwagi lub niedowagi, cz\u0119sto by\u0142y to tak\u017ce osoby w podesz\u0142ym wieku. Ograniczona aktywno\u015b\u0107 i id\u0105cy za ni\u0105 brak mo\u017cliwo\u015bci nawi\u0105zywania relacji spo\u0142ecznych, b\u0119d\u0105cych wa\u017cnym elementem terapii by\u0142 cz\u0119sto elementem uniemo\u017cliwiaj\u0105cym popraw\u0119 ich stanu zdrowia psychicznego. Umo\u017cliwienie wsparcia ww. os\u00f3b odpowiednio dobran\u0105 rehabilitacj\u0105 by\u0142o uzupe\u0142nieniem terapii i zwi\u0119ksza\u0142o szans\u0119 powrotu do aktywno\u015bci spo\u0142ecznej i zawodowej uczestnik\u00f3w. Zakres rehabilitacji by\u0142 ka\u017cdorazowo dobrany na podstawie wskaza\u0144 specjalist\u00f3w pracuj\u0105cych dotychczas z dan\u0105 osobom. W celu podniesienia jako\u015bci realizacji zadania zdecydowano o z\u0142o\u017ceniu oferty wsp\u00f3lnej, w jej celu podpisana zosta\u0142a UMOWA O OFERCIE WSP\u00d3LNEJ pomi\u0119dzy Fundacj\u0105 AKME a Fundacj\u0105 Reha+.
